Before the meeting:
1.
Prepare for the meeting by reviewing your notes on the employee's verbal warning conversation and any other pertinent material.
2.
Ask the employee to meet you in a private office, conference room or another private area.
During the meeting:

State the specific problem in terms of actual performance and desired performance.
4.
Refer to previous casual conversations and the previous verbal warning.
5.
Give the employee a chance to respond and explain.
6.
Tell the employee the specific change you expect in his or her performance and indicate your confidence that the employee will change.
7.
Have the employee confirm that he or she knows exactly what you expect.
8.
Tell the employee that you will write a memo to the file summarizing the conversation about the written warning.
After the meeting:

Write a short memo summarizing your conversation with the employee.
10.
Advise the appropriate people (department manager or Human Resources) of the conversation and distribute copies of the memo.
11.
Monitor the employee's performance to make sure that the problem has been corrected.
